Referral Process

It is helpful if you can provide the following information:
- Your relationship to the individual being referred (spouse, family member, partner, administrator, manager, etc.)
- That individual’s name
- The nature of your concern
- When you referred/will refer the individual, and if a deadline for their contacting NCPHP was established
- The nature of any present performance problems
- Documentation and the steps you have taken to correct the problem
If you are an administrator, Chief of Staff, practice manager, or hold a similar supervisory position, NCPHP recommends that you send written notice of the referral to the referrant with a copy to NCPHP, outlining the reasons for the referral. Prior to the written record, meet with the individual, if possible, and review your concerns. Explain NCPHP’s mission and emphasize that the services are confidential and professional. You can stress that NCPHP focuses on wellness and assessing mental health, medical, substance abuse, stress, and behavioral problems that could affect their ability to practice safely. Provide NCPHP contact information and encourage the individual to make an appointment.
