Does NCPHP need to know when I am going on vacation or other trips? Who should I notify?

YES. Be sure your assigned Field Coordinator is aware of your plans, and also advise NCPHP by e-mail or call 1-800-783-6792.

My doctor has prescribed a medication that is potentially habit-forming. What should I do?

Before filling the prescription, discuss the circumstances with your Field Coordinator and/or one of NCPHP’s medical directors. If deemed appropriate for you to take this medication, you should immediately turn it over to your spouse/signficant other, or other responsible party who will help control and monitor your dosage.

Who should I call if I have a change of address or telephone numbers, or change in employment?

Please advise your Field Coordinator and Kim McCallie at NCPHP.

I am experiencing financial difficulties and can’t pay my monitoring fees. What should I do?

Talk to Kim McCallie to discuss a payment plan or other arrangement.

I have been told I need to check into a treatment center, but am unable to gather sufficient funds. Can NCPHP help?

NCPHP has limited funds available to help in such situations. Call one of the medical directors to discuss a treatment scholarship. Unfortunately, NCPHP is not able to provide assistance for assessment costs.

I am going to inpatient treatment and want my spouse/family members to participate in the family program at my treatment center, but we cannot afford the expense. Can NCPHP help?

In honor of Dr. Michael Wilkerson, former NCPHP medical director, the Family Assistance Fund was established for this purpose. Grants are available to help with travel expense, child care, etc. Call Kim McCallie for details or to apply for a grant.

Is there an established support group for spouses/significant others of NCPHP program participants?

NCPHP is currently in the process of organizing a network of spousal support groups. For more information, or if you’d like to organize a group in your area, contact Nicole Trail at NCPHP.

Does NCPHP offer help with relapse prevention?

NCPHP field coordinators conduct relapse prevention seminars or workshops in various cities annually. Our Family Room library also contains professional videotapes about relapse prevention that you can borrow.

Who should I contact with questions about monitoring agreement requirements, urine drug screens, etc.?

Nicole Trail is NCPHP’s case management coordinator, and can help you with these aspects of your monitoring agreement.

What should I do if I am changing my therapist or discontinuing therapy?

Notify Nicole Trail and provide contact information for your new therapist. A Release of Information (link to download form) will need to be signed allowing your therapist to provide NCPHP with quarterly reports.

How can I obtain an advocacy letter for an insurance company, employer, etc.?

NCPHP can only share information about your involvement with the program if you have signed a Release of Information (link to form download). Once this release has been completed, fax (919-870-4484) or email Kim McCallie, and provide complete information regarding the intended recipient.
